Travel Home Health PT – 13 Week Contract | Days


We’re seeking an experienced Physical Therapist (PT) for a rewarding travel opportunity in beautiful Vermont!


Location: Newport, VT
⏳ Contract: 13 Weeks
Schedule: Monday–Friday | 8:00 AM–5:00 PM
⏰ Guaranteed Hours: 32 hrs/week
EMR: HealthWyse


Requirements:


✅ Active PT License
✅ Minimum 3+ years of PT experience
✅ OASIS experience required
✅ Home Health & Hospice experience required
✅ Experience traveling from patient to patient for home visits
✅ Ability to manage and adapt to changing schedules
✅ Strong communication and documentation skills


Responsibilities:


Provide physical therapy services in patients’ homes
Assess patient needs and develop individualized treatment plans
Deliver appropriate therapeutic interventions to improve mobility and function
Monitor patient progress and modify care plans as needed
Educate patients and caregivers on exercises and home-care strategies
Collaborate with healthcare professionals to support patient outcomes
Maintain accurate and timely clinical documentation
