Licensed Practical Nurse (Per-Diem)

Eminence Home Care CT is seeking a skilled and compassionate Licensed Practical Nurse to join our team. As a Per-Diem LPN, you will play a pivotal role in providing quality healthcare and compassionate support to our patients in the comfort of their homes. Responsibilities

• Administering medication as prescribed by the patient's physician in accordance with state regulations
• Performing basic patient assessments, including vital signs and monitoring health status changes
• Providing wound care and dressing changes, as needed
• Assisting patients with daily living activities, such as bathing, dressing, and grooming
• Educating patients and family members on proper healthcare management and disease prevention techniques
• Collaborating with the patient's physician and other healthcare professionals to develop a care plan that meets the unique needs of each patient
• Maintaining detailed and accurate patient records and documentation

Requirements

• Active LPN license in good standing
• Minimum of 1-year experience in a clinical setting, preferably in home health care
• Valid driver's license and reliable transportation
• Ability to work independently and make sound decisions based on patient needs
• Strong communication and interpersonal skills, with the ability to work with a diverse patient population
• Knowledge of state and federal regulations governing home health care
• CPR certification and basic life support training
