Job Summary:
Evercare of Collinsville is seeking a compassionate and skilled LPN Licensed Practical Nurse to join our team of dedicated healthcare professionals. Responsibilities:
Provide direct patient care under the supervision of a registered nurse, including administering medications, wound care, and vital signs
Develop and implement individualized care plans in collaboration with the interdisciplinary team
Monitor and report patient progress, including any changes or concerns
Maintain accurate and detailed records of patient care and treatment
Collaborate with other healthcare professionals to ensure seamless transitions and continuity of care
Participate in quality improvement initiatives and educational programs to stay current with best practices

Requirements:
Current LPN licensure in the state of Illinois
Minimum of 0 years of experience as an LPN (recent graduates encouraged to apply)
Ability to work in a fast-paced environment with frequent interruptions
Excellent communication and interpersonal skills
Ability to lift, transfer, and reposition patients as needed
