Travel Home Health Physical Therapist 

We are seeking an experienced and compassionate travel Physical Therapist to join a growing Home Health team. In this role, you will provide skilled therapy services to patients in their homes, helping them regain mobility, improve function, and achieve their highest level of independence. Clinicians will manage a variety of patient populations and perform comprehensive evaluations and ongoing treatment planning.

Job Description & Requirements

• Specialty: Physical Therapist- Home Health
• Years of Experience: 1 year
• Home Health experience preferred
• Experience with Homecare Homebase (HCHB) preferred
• Strong clinical assessment and documentation skills
• New graduates will be considered, although prior clinical experience is preferre
• Certifications:
  • BLS Certification
  • Active KY license (Compact Accepted) 

Schedule: Monday–Friday, 8:00 AM–5:00 PM, with rotating weekends

Responsibilities

• Perform Start of Care (SOC), Resumption of Care (ROC), Recertifications, Evaluations, and routine patient visits
• Develop and implement individualized treatment plans based on patient needs and physician orders
• Assess patient progress and modify treatment plans as appropriate
• Educate patients and caregivers on exercises, safety, and home programs
• Maintain timely and accurate documentation within the electronic medical record
• Collaborate with interdisciplinary healthcare team members to ensure quality patient care
