Roles & Responsibilities
- Evaluate and treat patients with physical impairments in our 74-bed skilled nursing facility
- Develop individualized care plans to restore function and improve mobility
- Collaborate with the interdisciplinary team to ensure comprehensive care
- Educate patients and families on therapeutic exercises and techniques
- Document patient progress accurately and timely
- Participate in care conferences and coordinate discharge planning
Qualifications
- Current New Jersey Physical Therapist license
- DPT or Master's degree in Physical Therapy (new graduates welcome)
- Experience in skilled nursing or long-term care is preferred
- Strong clinical and communication skills
- A passion for improving quality of life for elderly patients
Knowledge & Skills
- Expertise in rehabilitation techniques and therapeutic exercise
- Ability to perform patient assessments and create treatment plans
- Knowledge of Medicare and insurance documentation requirements
- Excellent interpersonal and teamwork skills
- Proficiency with electronic health records
