The NP will assess, diagnose, and manage acute and chronic conditions while collaborating with an interdisciplinary care team to improve patient outcomes and quality of life.Key ResponsibilitiesPerform comprehensive health assessments, diagnose and manage acute and chronic conditions, and develop individualized, evidence-based care plans for residents in a skilled nursing settingProvide ongoing medical management including ordering and interpreting diagnostics, prescribing treatments, conducting medication reviews, and proactively monitoring conditions to prevent complications and reduce hospitalizationsDeliver timely clinical interventions in response to changes in patient status, ensuring high-quality, patient-centered care that improves outcomes and supports functional well-beingSupport value-based care initiatives by focusing on quality outcomes, reducing avoidable hospital readmissions, optimizing cost of care, and utilizing data to guide clinical decision-making and performance improvementCollaborate closely with physicians and interdisciplinary team members to ensure coordinated, efficient care, and participate in care plan meetings and quality improvement effortsFacilitate smooth transitions of care across settings, ensuring continuity, reducing gaps in treatment, and supporting successful admissions and dischargesEducate residents and families on diagnoses, treatment plans, medications, and preventative care strategies to promote engagement and long-term healthMaintain accurate, timely, and compliant documentation while adhering to all clinical, regulatory, and organizational standards
