Restorative Nurse ( LPN )

Lineville Health & Rehabilitation, LLC is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our team as a Restorative Nurse . In this role, you will help residents maintain and improve their highest level of physical, mental, and psychosocial well-being through individualized restorative nursing programs. Responsibilities Coordinate and oversee restorative nursing programs for assigned residents. Collaborate with nursing staff, therapy services, physicians, residents, and families to develop and implement restorative care plans. Participate in the MDS/RAI process and interdisciplinary care planning. Monitor resident progress and communicate changes or concerns to the appropriate healthcare providers. Initiate therapy referrals and serve as a liaison between the nursing and therapy departments. Ensure restorative services are delivered in accordance with physician orders, resident goals, and facility policies. Educate residents and families regarding restorative care programs and goals. Maintain accurate documentation and compliance with all federal and state regulations. Qualifications Current LPN license in good standing. Knowledge of restorative nursing principles and long-term care practices preferred. Strong communication, leadership, and organizational skills. Ability to work collaboratively with an interdisciplinary team. Compassionate, patient-centered approach to resident care.
