Job Description
Title : Physical Therapist - Home Health

Schedule :
- Shift: 0700-1530
- Days: Monday to Friday
- Type: Day Shift

Assignment Details :
- Contract Length: 13 weeks
- Guaranteed Hours: Not specified

Requirements :
- Degree: Doctor of Physical Therapy (DPT) required
- Certification: AHA BLS required
- Experience: Minimum of 1 year in home health
- EMR: Knowledge of HCHB and OASIS

Roles & Responsibilities :
- Evaluate and create treatment plans for patients in a home health setting.
- Implement therapeutic interventions to help patients improve their mobility and function.
- Educate patients and families on treatment plans and home exercises.
- Document patient progress and communicate with healthcare team members.
- Ensure compliance with applicable laws and regulations in home healthcare.
- Provide compassionate care and support to patients and families.

Compliance / Notes :
- Mileage reimbursed per government rate.
- No weekends or call required.
- Admin fee deducted from rate.
