As a LPN (Licensed Practical Nurse) , you will play a vital role in delivering individualized care programs that cater to the unique needs of each patient.
Responsibilities:
Provide skilled nursing care to patients and residents under the supervision of a registered nurse or a licensed physician
Administer medications, injections, and treatments as prescribed by physicians
Conduct patient assessments and report on their condition to healthcare teams
Develop and implement personalized care plans to achieve patient goals and outcomes
Maintain accurate and up-to-date patient records and files
Collaborate with other healthcare professionals to ensure seamless patient care
Participate in quality improvement initiatives to advance patient care and safety
