Licensed Vocational Nurse Job Title: Licensed Vocational Nurse Profession: LVN Specialty: Home Health Duration: 13 weeks Shift: Day Shift Hours per Shift: 8 hours Experience: Minimum experience required License: Licensed Vocational Nurse (LVN) licensure in the state of practice Certifications: Basic Life Support (BLS) Health Care Provider certification Must-Have: Valid Driver's License and must be 21 years of age or older Description: Our home health and hospice agencies serve the entire community and offer personalized in-home care. The position involves performing procedures that require specialized training for general nursing care and related services for assigned patients. You will work under the direction of a Registered Nurse or Medical Doctor, following established policy. Participation in the planning and provision of patient care is expected. You will use knowledge of asepsis in the performance of treatments, such as sterile dressings and cauterizations. Functional direction will be provided to ancillary personnel. On-call duties will be assigned based on operational needs. If assigned, you will be required to perform on-call duties. This role includes providing care for assigned patients according to established standards and nursing processes. Routine visits will be performed based on departmental specific policies and procedures. You will assist in the development of a prioritized interdisciplinary plan of care for each patient. Implementation and coordination of the plan of care will utilize appropriate organizational skills and reporting mechanisms. Individual teaching plans will be implemented, emphasizing patient decision-making and mutual goal setting. Assigned clinical procedures, including phlebotomy, injections, and drug screens, will be performed. Medication administration will be your responsibility. You will order and restock supplies as assigned. Maintaining cleanliness in assigned areas is necessary. Other job-related duties may be assigned as needed.
