Patient Care Coordinator

The Patient Care Coordinator oversees patient care management for the acute inpatient population, collaborating with physicians, nursing staff, and other healthcare team members to ensure seamless care transitions and cost-effective outcomes. Key Responsibilities Include Conducting initial assessments within 24 hours of admission to identify discharge needs and developing transition plans. Participating in daily rounds and documenting care progression, discharge plans, and barriers. Coordinating with social workers for long‑term care, financial assistance, and community services. Managing referrals for post-acute care and escalating delays or barriers to care. Ensuring follow-up appointments and referrals are scheduled before discharge. Adhering to safety policies, participating in quality assurance, and maintaining professionalism. Working collaboratively with patients, families, staff, and external providers to optimize patient outcomes. Qualifications Include Minimum one year of experience in acute care nursing (medical, surgical, ED, or critical care). Associate Degree in Nursing required; BSN preferred. Registered Nurse license in applicable states or compact license within six months. Basic Life Support certification. Experience with electronic medical records, healthcare reimbursement systems, and strong communication skills. Ability to work independently with critical thinking, time management, and teamwork skills.
