About This Role The Registered Nurse in the Care Transition Center at Hackensack Meridian Health is responsible for providing expert clinical support and education to team members, patients, and the community. Highlights Location: Neptune, NJ Facility: Hackensack Meridian Health Schedule: Per Diem, Day shift What You'll Do Function as an expert clinical nurse resource assisting team members, physicians, patients, and the community. Act as a role model by demonstrating current trends and providing professional development opportunities. Independently develop, implement, and evaluate educational plans for team members. Actively participate in the research process to enhance patient care. Function in the role of clinical nurse specialist to nursing staff, administrators, and physicians. Facilitate patient teaching and support programs utilizing the nursing process. Requirements Education: BSN in progress and/or willing to acquire within three (3) years of hire Experience: At least 3 years of recent critical care or telemetry experience strongly preferred Licenses & Certifications: Current and valid New Jersey Registered Nurse license required BLS/CPR certification by American Heart Association required within 60 days of hire ACLS required About Hackensack Meridian Health Hackensack Meridian Health is a leading healthcare provider committed to delivering exceptional patient care.
