About the Role Physical Therapist (PT) – Provide rehabilitative care to residents in short‑term rehab and long‑term care settings at CareOne at Peabody, MA. Position Information Location: Peabody, MA Facility: CareOne at Peabody Practice Setting: Short‑Term Rehab & Long‑Term Care Compensation: $40.00 – $65.00 per hour (based on experience, qualifications, credentials, employment status, and work schedule) Essential Duties and Responsibilities Provide and direct resident rehabilitation care within accepted practice methods, meeting all center, state, federal, and accrediting standards. Evaluate resident needs and establish effective treatment plans. Coordinate supervision of treatment plans with staff and other disciplines. Participate in screening of residents per directed schedules. Provide feedback to supervisors on clinical and rehabilitation issues. Participate in center‑directed processes and services. Maintain clinical documentation to meet regulatory requirements. Maintain accurate billing records. Participate in in‑service training programs. Travel to other facilities on a limited basis if needed. Position Requirements Master’s or Doctorate Degree in Physical Therapy. Current and active PT license in Massachusetts. Experience or knowledge in older adult rehabilitation preferred. Knowledge of government and regulatory guidelines in designated healthcare settings preferred.
