Case Manager (QIDP)

Location: Main Campus (primarily), group homes, and other locations Reports To: Client Care Manager FLSA Status: Hourly/Non-exempt Employee Status: Full-Time Work Schedule: Regular business hours Monday to Friday and other hours intermittently or as assigned Supervision Given: Residential Direct Support Professionals (DSPs) (functional supervision) Supervision Received: Client Care Manager(s), Executive Director Nature and Scope of Position A Case Manager, under the direct supervision of a Client Care Manager, is a member of the interdisciplinary team and responsible for holistic programming and individual service plans (ISP) for an assigned caseload for Magnolia residential clients – adults with intellectual and/or developmental disabilities. A Case Manager assists with managing the group home operations and daily living activities of their clients in compliance with their individual ISPs and behavior support plans (BSP), if applicable, and provides functional supervision to assigned DSPs. Primary Qualifications Education Bachelor’s Degree (B.A. or B.S.) in a professional category that satisfies the applicable regulations. Work Experience At least one year of experience working directly with persons with intellectual disability or other developmental disabilities. Certification or License Licensing requirements vary depending on how the Case Manager will be designated (e.g., social worker, human services professional). Current Automotive Insurance Coverage required Attendance at Mandatory Training & In-Services Current CPR certification. Special Skills Excellent verbal and written communication skills. Excellent organization and time management skills. Ability to prioritize responsibilities. Capacity to make decisions using sound judgment. Willing to seek guidance in new or unusual circumstances. Ability to stay calm in difficult and emotional situations. Basic technology skills appropriate to complete and review relevant documentation and manage email. Ability to present information in one-on-one and small group settings. Ability to develop knowledge of relevant Medicaid and ICF/IID rules and relevant licensure standards shortly after employment. Other Considerations 21 years of age or older. Valid, unencumbered Louisiana driver’s license. Current automotive insurance coverage with acceptable limits as mandated by Magnolia’s insurance carrier. Satisfactory Motor Vehicle Record. Ability to pass background screening including pre-employment drug test, physical, background check, and other requirements. Ability to be reached for assistance when emergencies arise. Weekend, evening, late-night work, and overtime required at times. Job Functions Essential Duties and Responsibilities Essential Duties and Responsibilities: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Individual Service Plan (ISP) Develops annual ISPs, as a member of the interdisciplinary team, to address goals and objectives designed to increase independence and self-sufficiency in areas of daily life. Supervises implementation and integrates the various aspects of the ISP. Records the client’s progress as measured by objective indicators. Continuously monitors the ISP and proposes revisions as necessary. Reviews the ISP on a quarterly basis and prepares quarterly status reports on the progress of the client relative to the goals and objectives of the ISP. Client Care Shows utmost respect for people with disabilities and their families. Uses person-centered planning principles for goal development and service planning, incorporating both what is important to and important for the client. Engages with clients regularly and learns about each client’s specific needs to plan for and coordinate the delivery of services to meet those needs and to facilitate the acquisition of skills and adaptive behaviors for each client. Serves as an advocate for the client when assistance is needed or services are not being performed as required. Ensures the coordination of quality client care and manages internal and external services and supports in accordance with the stated goals and objectives of every ISP. Communicates all client medical problems to the wellness team, and regularly checks in with the wellness team regarding client health and wellness. Uses creativity in developing holistic wellness supports and monitors client progress. Transports client to and actively participates in 90L annual appointments and other medical appointments as needed. Reviews and investigates incident reports and other incidents pursuant to policy. Acquires and implements services, equipment, programs, and supports according to doctors’ orders and/or decisions made by the interdisciplinary team. Accurately and timely completes and regularly reviews ICAPs for assigned caseload. Maintains petty cash for clients on caseload, keeps current and accurate records of expenditures, and adheres to Magnolia’s policies and procedures and they relate to handling and securing funds. Monitors client account balances and keeps client funds under $2,000. Provides training in budgeting, shopping, and money management as appropriate. Timely submits complete expense reports with accompanying receipts. Learns about client behaviors, implements appropriate de-escalation techniques, and trains staff on same. Arranges transportation for clients with jobs off campus. Group Home Management Provides a home-like, client-centered atmosphere using appropriate techniques in training and positive approach to behavior management. Conducts routine group home inspections for safety, cleanliness, adequate food supplies, dietary compliance, and quality of life areas, reports perceived deficiencies pursuant to Magnolia procedure, and submits timely maintenance work orders. Reports maintenance needs in vehicle assigned to or used by group home. Maintains accurate and up to date drills, inspections, and other documentation. Serves as primary point of contact for surveys, inspections, and ombudsman issues. Functional Supervision of DSPs Provides functional supervision to DSPs in the delivery of support for clients served. Interacts with DSPs in a respectful way. Participates in ongoing DSP development and training in both formal and informal settings on the operations of their assigned group homes, client care (including client rights, ISPs, BSPs, dietary restrictions, medical orders, proper feeding techniques, training objectives, and personal care), and other pertinent areas. Ensures DSPs document appropriately and timely in Residex and other systems. Encourages information sharing among team members. Family Engagement Serves as main point of contact and liaison for families and, if appropriate, legally responsible parties to address concerns, answer questions, and relay information. Coordinates client home visits, transfers and discharges. Fosters ongoing positive communication and contact between individuals and their families, their friends, and other significant people in their lives. Maintains confidentiality with all clients and their families. Other Functions Understands and complies with Magnolia’s mission, vision, and values. Works as a team with other Case Managers and Magnolia staff, stepping in to assist or transport other clients and staff as needed. Maintains a professional image as a representative of Magnolia. May need to evacuate with Magnolia clients and staff in emergency situations. Performs other related duties as assigned. #J-18808-Ljbffr Magnolia Community Services
