Home Care Service Coordinator (Scheduler)

Ambercare is currently seeking a Full Time Service Coordinator based in Albuquerque, NM. The Service Coordinator will schedule and supervise in‑home care workers and clients, conduct home visits to monitor quality of care and determine whether care plans are adequate. Location Ambercare, 2129 Osuna Rd. NE, Albuquerque, NM 87113 Schedule Monday–Friday 8:00 AM–5:00 PM, with occasional evenings and weekends as needed. Benefits Medical, Dental, and Vision coverage Company‑matched 401(k) Continued education opportunities Monthly agency bonus potential PTO plan Retirement planning Life insurance Employee discounts Essential Duties Creates work schedules in the system, manages changes due to client request, illness, vacation, or leaves of absence. Provides alternate coverage to ensure the care plan is followed and client services are not interrupted. Contacts care providers and clients to provide service updates. Monitors client condition and performance of duties by Home Care Aides. Maintains all company and Department on Aging rules, regulations, and standards. Conducts home visits to new clients to welcome them and review the Welcome Packet. Observes and evaluates Home Care Aides in the client’s home as directed by Supervisor to assure Care Plans are being followed. Prepares and completes accurate evaluation reports and case notes as needed. Maintains schedules to assure timely completion of all assigned home visits. Attends weekly branch meetings. Maintains confidentiality at all times due to access to sensitive information. Maintains regular, predictable, consistent attendance and is flexible to meet the needs of the department. Follows all Medicare, Medicaid, and HIPAA regulations and requirements. Abides by all regulations, policies, procedures, and standards. Performs other duties as assigned. Position Requirements & Competencies Minimum of 3 years’ experience in senior services or a related social services field. Excellent oral and written communication skills. Strong interpersonal skills. Ability to prioritize and handle high call volumes. Client‑focused and customer service‑driven. Proficient in Microsoft Word, Excel, and Outlook. Must have reliable transportation, a valid driver’s license and insurance for travel within the branch service area.
