Work Environment

In this role, you will work primarily in a home setting, bringing your expertise directly to patients' residences. You will join our growing team of rehabilitation professionals who provide care in an atmosphere of support and collaboration. While our base operations are grounded at our state-of-the-art rehabilitation facility, the flexibility of homecare allows you to tailor treatment plans to meet patients' specific needs while fostering relationships that enhance their recovery experience.

Key Responsibilities

• Provide personalized, evidence-based orthopedic therapy in the homes of patients recovering from injuries, surgeries, or chronic musculoskeletal conditions.
• Assess each patient's safety, mobility, and functional capabilities in their home environment, adapting interventions to ensure effective rehabilitation.
• Collaborate with Orthopedic MDs, Occupational Therapists, Chiropractors, and outpatient Physical Therapists to develop and implement integrated care plans that cater to each patient's recovery journey.
• Educate patients and their families about exercises, body mechanics, and adaptive strategies that will empower them throughout their rehabilitation.
• Maintain accurate and secure documentation of patient sessions using user-friendly electronic medical record (EMR) tools.
• Engage in interdisciplinary case reviews and clinical education to foster professional development and improve patient outcomes.

Patient Population / Client Focus

You will work with a diverse patient population, including individuals recovering from orthopedic surgeries, traumatic injuries, and chronic pain due to musculoskeletal disorders. This role will allow you to make a direct impact in each patient's home by delivering compassionate care that promotes independence and enhances their quality of life.

Qualifications

• Valid Physical Therapist license in the State of California (or eligibility to obtain one).
• A Bachelor's Degree in Physical Therapy is required.
• At least two years of experience in a physical therapy setting, particularly with a focus on orthopedic care.
• Strong communication and organizational skills to effectively manage patient care and responsibilities.
• Ability to work independently while managing a homecare caseload with flexibility.
• A genuine commitment to personalized patient-centered care.

Preferred Experience

• Previous experience in a homecare or outpatient orthopedic setting is preferred, but confident new graduates are encouraged to apply.
• Familiarity with electronic medical record (EMR) systems for seamless documentation and communication.

Compensation and Benefits

We offer a competitive salary ranging from $90,000 to $120,000 per year, commensurate with experience and qualifications. Additionally, our benefits package includes:
