RN Diabetes Patient Educator

About This Role As an RN Diabetes Patient Educator at Cleveland Clinic Weston Hospital, you will coordinate the diabetes education program, provide educational resources, and facilitate organizational change to improve patient care. This role is ideal for those passionate about diabetes management and patient education. Highlights Location: Weston, FL Facility: Cleveland Clinic Weston Hospital Schedule: Full-time, Day shift What You'll Do Provide direct patient care using medical nutrition therapy or diabetes self-management education. Collaborate with Endocrinology providers to design and administer nutrition and diabetes care. Complete comprehensive assessments for each patient, including emotional and behavioral health. Develop individualized care plans with safe and effective interventions. Utilize national standards and guidelines for diabetes and obesity care. Use diabetes and weight management technology to assess status and track outcomes. Deliver specialized interventions and comprehensive counseling services. Implement care plans, communicate plans, and maintain documentation. Provide timely documentation of each patient encounter in the electronic medical record. Develop and contribute to education programs for healthcare professionals and the public. Requirements Education: Bachelor's degree or higher in Nursing, Dietetics, Pharmacy, or a closely related field Experience: One year of experience as a Registered Dietitian, Registered Nurse, or Pharmacist Licenses & Certifications: Current license to practice dietetics, nursing, or as a Pharmacist in the State of Florida Active certification as a Registered Dietitian/Nutritionist (RD/RDN) for Dietitians Certification as a Certified Diabetes Care and Education Specialist (CDCES) within two years of hire/assignment About Cleveland Clinic Cleveland Clinic is a leading healthcare provider committed to delivering exceptional patient care.
