Job Description
We are looking for a compassionate Home Health Aide to provide assistance in daily living and personal care services of residents of Brooklyn Adult Care Center in accordance with an established care plan.
Responsibilities
- Provide health care services in the facility
- Perform personal care tasks
- Accompany patients to doctors office, on-site and off-site
- Assist with clients personal care activities
- Monitor patients and report on their condition to nurses
- Reporting and documenting of patients care and provided services
- Assist patients with ambulation and transferring
- Provide companionship and basic support
Skills
- Proven working experience as home health aide
- Ability in following basic care instructions from supervisors and nurses
- Proficiency in English; other languages, a plus
- Caring and compassionate personality
- Able to work in a team
- Current HHA certificate
- Valid government ID
Job Types: Full-time, Part-time
Salary: $15.00 /hour
