Step into a dynamic field-based role where you'll make a meaningful impact in the lives of patients by delivering skilled physical therapy services directly in their homes. This position prioritizes personalized, patient-centered care aimed at improving mobility, strength, safety, and overall independence.

Schedule:

• Full-time, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM
• Approximately 2532 points per week (generally 57 patient visits per day depending on visit type)

Responsibilities:

• Perform comprehensive physical therapy evaluations, including OASIS assessments for start of care, resumption, recertification, and discharge visits where applicable
• Develop and implement individualized plans of care focused on restoring function, promoting safety, and managing pain
• Deliver skilled therapies such as therapeutic exercise, neuromuscular re-education, gait and transfer training, and fall prevention strategies
• Continuously monitor and document patient progress, promptly updating care plans and communicating with physicians and the interdisciplinary team as needed
• Educate patients and caregivers on exercise programs, safety strategies, and the use of assistive devices to optimize outcomes at home
• Accurately complete timely documentation using Home Care Home Base (HCHB)
• Actively participate in interdisciplinary meetings, case conferences, and ongoing education to ensure a collaborative approach to care
• Adhere to all regulatory, agency, and Medicare home health standards

Qualifications:

• Active, unencumbered Physical Therapist (PT) license in the state of practice
• At least one year of recent Home Health PT experience (field-based background highly valued)
• Required experience with OASIS assessments; proficiency in Home Care Home Base (HCHB) preferred
• Strong clinical assessment, time management, and organizational skills
• Reliable transportation, valid driver's license, and current auto insurance

$2,134 - $2,258/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
