RN Case Manager

Good Samaritan Health Center seeks a mission-driven RN Case Manager to coordinate holistic, patient-centered care for underserved individuals and families in Atlanta. In this role, you will perform comprehensive assessments, develop and manage care plans, and guide patients through complex medical, behavioral health, and social needs across settings. You will collaborate closely with an interdisciplinary team, facilitate transitions of care, address barriers like cost and access, and connect patients to community resources.
