JOB DESCRIPTION

This is an inpatient position within St Mary's hospital Rehabilitation Services department. Job Summary This is an inpatient position within St Mary's hospital Rehabilitation Services department. The Per Diem Physical Therapist reports to the Clinical Supervisor and provides both direct and indirect patient care services. The Per Diem Physical Therapist works primarily with adult and geriatric patients from diverse socioeconomic and cultural backgrounds. The Per Diem Physical Therapist with specific pediatric competencies and clinical experience may also work with infants, children and/or adolescents. Under general direction and upon receipt of licensed approved provider written referrals, the Per Diem Physical Therapist plans and administers a full range of physical therapy services for patients with a variety of physical and cognitive impairments and disabilities. Services include patient evaluations using physical therapy tests and measurements, the establishment of therapeutic intervention goals and the development of therapeutic intervention programs, and the implementation of therapeutic intervention plans utilizing physical therapy interventions, modalities, and procedures. The Per Diem Physical Therapist maintains all applicable patient records related to the delivery of patient care services. While performing job duties, the Per Diem P.T. may also supervise support personnel including Physical Therapist Assistants and Rehabilitation Aides. As a member of the clinical care team, the Per Diem Physical Therapist also participates in compliance, quality, safety, and process improvement initiatives in the department and the hospital. The Per Diem Physical Therapist minimum work requirement is two (2) weekend day shifts per month, and one (1) major and one (1) minor Winter UC Holiday (i.e.,Thanksgiving/Day After, Dec24/25, Dec 31/Jan). Responsibilities Identify the functions or tasks that employees in the job perform. The essential functions should state the purpose of the work and the results to be accomplished, rather than how the function is performed. Of the tasks listed, what percentage of time is devoted to each? The more time employees spend on a function, the more likely it is that the function is essential. Generally, include those functions that account for 10% or more of the work, i.e., key items that contribute significantly to the achievement of the job. The functions should add up to 100%. Direct and Indirect Patient Care (85%) Performs physical therapy evaluations on referral by a licensed approved provider and administers various muscle, neurological, gait, functional and developmental tests to determine a patient’s functional impairments. Establishes short- and long-term therapeutic goals based on results of the evaluation and in consultation with the referring provider and caregiver/family. Develops a therapeutic plan of care based on results of the evaluation and established goals. Makes appropriate changes in therapeutic interventions based on the patient’s progress and performance. Administers direct patient care utilizing the full range of physical therapy techniques and methods within scope of practice and level of competence, appropriate to the patient’s goals and plan of care. Delegates and supervises any part of the patient’s therapeutic intervention which can be legally and appropriately done by support staff. Completes all aspects of discharge planning as appropriate. Prepares and maintains accurate and timely patient care records as required by policy and payer requirements, including services rendered, progress notes, discharge notes, charges for services provided, and third-party payer reports (eg CCS, Medi-Cal). Communicates effectively and efficiently with patients, caregivers/families, department staff, managers, and other clinical personnel. Reports on patient progress at various rounds, clinics, and team conferences as indicated. Demonstrates effective time management and organization skills. Performs any other duties related to patient care, as required. Departmental Responsibilities (15%) Completes work-related routine departmental tasks (eg. timecard completion, schedule sign up) in a timely manner. Communicates clearly and timely with Supervisor and Manager regarding work requirements including changes in availability/status, conformance with PT licensure and AHA BLS certification renewal, and completion of UC required learning. Participates in and supports department and organizational compliance, quality, safety, and process improvement activities. Performs any other duties, as assigned by the Supervisor or Manager. Represents the department as a professional. Qualities & Requirements Required Qualifications DPT Doctoral graduate degree If graduated 15 or more years ago, a bachelor’s degree Minimum of one year of related patient care experience. Ability to communicate effectively in English, both orally and in writing. Ability to recognize signs of distress. Basic computer skills including work within Epic electronic medical record Possession of good interpersonal, time management, problem solving and self-initiation skills. This position requires flexibility to orient and work at all UCSF Medical Center locations. Preferred Qualification Experience providing patient care in acute care hospitals and acute rehab/IRF patient care settings. Required Licenses/Certifications Possession of or proven eligibility for licensure as a Physical Therapist by the Physical Therapy Board of California. Possession of or proven eligibility for AHA BLS certification.
