Care Coordinator

Location: Troy, MI, with travel to Utica, MI and client appointments
Compensation: $45,000–$60,000 annually
Schedule: Full-Time, Monday–Friday, 40 hours per week

Position Summary

Our client is seeking a compassionate, organized, and detail-oriented Care Coordinator to oversee clinical case management and coordinate services for individuals recovering from acquired brain injuries (ABI). This role serves as a liaison between clients, families, medical providers, internal clinical teams, and external stakeholders to ensure treatment plans are followed, appointments and services are coordinated, and client progress is accurately documented.

The ideal candidate has experience in healthcare coordination, case management, patient advocacy, or social services, along with strong communication, organizational, and problem-solving skills. This position is based in Troy, MI, with regular travel to the Utica location, client homes, and medical appointments.

Key Responsibilities

• Coordinate and facilitate medical care, treatment plans, appointments, transportation, and related services in accordance with physician orders.

• Attend medical appointments with clients and communicate updates regarding treatment progress, medications, concerns, and program participation to healthcare providers.

• Participate in weekly operational and clinical meetings, as well as monthly treatment team meetings, to discuss client progress, treatment goals, and care needs.

• Maintain accurate and timely clinical documentation, including progress notes, utilization reviews, monthly reports, medication updates, appointment schedules, and stakeholder communications.

• Coordinate with site supervisors, life skills staff, clinicians, administrative personnel, and external case managers to address clients’ daily medical and support needs.

• Advocate for clients by communicating their concerns, preferences, and needs to internal teams, healthcare providers, families, and external stakeholders.

• Identify and coordinate community resources, including financial assistance, legal aid, housing, employment, education, and additional treatment services.

• Monitor therapy schedules and service delivery to ensure alignment with established treatment plans and program goals.

• Provide emotional support and guidance to clients and families navigating acquired brain injuries, behavioral or cognitive challenges, court proceedings, and changes in benefits.

• Maintain compliance with HIPAA requirements and ensure required releases of information are current.

• Provide transportation to medical appointments and other services as needed.

• Maintain communication availability via company-issued cell phone to assist with client-related needs outside scheduled hours when necessary.

• Perform other duties as assigned.

Qualifications

• Experience in healthcare coordination, clinical case management, social services, rehabilitation, or a related field preferred.

• Strong organizational, documentation, communication, and interpersonal skills.

• Ability to coordinate multiple appointments, treatment plans, and competing priorities.

• Demonstrated ability to advocate for clients and collaborate with multidisciplinary teams.

• Knowledge of community resources, healthcare benefits, insurance processes, or Michigan auto no-fault insurance preferred.

• Experience working with individuals with acquired brain injuries, cognitive impairments, behavioral challenges, or dual diagnoses is an advantage.

• Ability to maintain confidentiality and follow HIPAA guidelines.

• Valid driver's license and ability to travel between locations and transport clients as needed.

Working Conditions

• Full-time, Monday–Friday schedule, 40 hours per week, with a paid half-hour lunch period.

• Regular travel between Troy and Utica, client homes, and medical appointments.

• Frequent interaction with clients, healthcare providers, staff, and external stakeholders.

• Ability to manage changing priorities and perform routine office tasks, including computer work, phone communication, filing, and driving.

• Ability to lift up to 25 pounds.
