Patient Diagnoses: - Special Procedures/Unit Details: - Special Equipment: - #Tier2 Travel Compliance Required for Submission: Submit candidates to a maximum of 3 openings and include: Candidate's top 3 job choices. Openness to other Northern Arizona Healthcare facilities, units, or shifts. RTO must be disclosed and approved at time of submission. Up to 5 non-holiday RTO days may be auto-approved for ICU, MS/Tele, and PCU specialties. Additional RTO requires facility approval. No RTO permitted during the first week of assignment due to orientation. Candidate must work 2 of the facility's recognized holidays. Medical Assistants must have: National certification. Arizona Fingerprint Clearance Card.

Submission Rules: Previous Northern Arizona Healthcare PERM, PRN, or EP staff must be separated for at least 12 months before returning as a traveler. Include the facility name and dates of employment in the submission packet. Rehire approval is required before an offer can be extended. Return travelers are ineligible if they canceled a previous NAH contract within 10 days of the scheduled start date. Candidates must be comfortable floating without restrictions. Floating may occur within the facility, to nearby affiliate hospitals, or to other units based on skills and census needs. Refer to the job description for specialty-specific floating requirements. Travel pairs are accepted on all units except ER. Travel pair status must be clearly identified at submission. Cerner experience is preferred/required based on the specific job posting. References may be pending at submission.

Start / Facility Instructions: Compliance Requirements: Candidate must complete lab work within 72 hours of contract signing. All compliance testing must be completed within 48 hours of assignment. Drug screen must be completed at least 30 days prior to start date. Compliance documents are due by the Wednesday prior to start. Two professional references are required prior to start: One reference must be from the most recent employer and completed within 1 year of the start date. The second reference must be completed within 2 years of the start date. Candidates who have lived or worked in the following locations should complete required state release forms as soon as possible to avoid background check delays: Georgia (GA) Nevada (NV) New Hampshire (NH) Massachusetts (MA) Puerto Rico U.S. Virgin Islands International background searches are not required. Holiday Requirements Candidates must be available to work 2 of the following holidays: New Year's Day Memorial Day Weekend Independence Day Weekend (July 4th) Thanksgiving Day Christmas Day Modules Modules are completed during the first week of orientation. All module hours are billable. Average module time is approximately 6 hours. Module completion is tracked via LMS transcripts.
