This role involves working in a home hospice environment, where you will provide compassionate end-of-life care. The position requires effective patient assessments, utilization management, and case management skills.

Key Responsibilities of the RN - Hospice - Travel

• Accurately assess patients for hospice care appropriateness.
• Develop and evaluate plans of care for patients and families.
• Maintain visit productivity standards and knowledge of insurance reimbursement.
• Perform and document patient education and discharge assessments.
• Assume responsibility for case management and interdisciplinary communication.
• Ensure patient and co-worker safety and follow infection control measures.
• Participate in Performance Improvement and agency business development activities.
• Attend scheduled staff and interdisciplinary team meetings.

Requirements of the RN - Hospice - Travel

• Two (2) years of experience as an RN in a home hospice environment.
• Current New Jersey RN License.
• Must have valid New Jersey driver's license.
• BLS certification required.
• Experience with EMR systems and strong critical thinking and communication skills.
