Outpatient Physical Therapist

This is a month-long contract position with the potential for extension.

5x8.0 work schedule.

The Outpatient Physical Therapist is responsible for evaluating assigned patients and developing, implementing, and modifying treatment plans based on the patient's needs and goals, in line with physician direction. This role requires the delegation and direction of therapy services, focusing on creating effective treatment programs to facilitate the patient's return to community activities.

Responsibilities:

• Conduct comprehensive evaluations of assigned patients.
• Develop and implement individualized treatment plans and goals to progress patients toward discharge.
• Maintain accurate and timely medical record documentation, ensuring compliance with all state and federal guidelines.
• Provide patient education and serve as a mentor for students and coworkers.
• Participate in performance improvement initiatives to enhance quality of services.
• Adhere to discipline-specific regulations and maintain professional licensure in good standing.

Required Experience / Certifications / Licensure:

• Degree in Physical Therapy from an accredited institution.
• 6 months to 1+ year of outpatient clinical experience.
• Current CPR certification.
• Active license for the state where work will be performed.
• Previous experience with Select Medical is a plus but not required.
