How you’ll make an impact in this role

• Holistic Care Coordination: Serves as the primary liaison across multidisciplinary teams to implement, monitor, and adapt comprehensive patient care plans in alignment with organizational protocols.

• Clinical Excellence & Monitoring: Executes complex physician and nursing orders with precision, maintaining rigorous oversight of patient conditions and ensuring timely, accurate documentation and communication.

• Strategic Discharge Planning: Leads the assessment and coordination of patient discharge requirements, collaborating with the healthcare team to ensure seamless transitions and continuity of care.

What minimum requirements you’ll need

Licensure / Certification / Registration:

• Registered Nurse credentialed from the Texas Board of Nursing obtained prior to hire date or job transfer date required.
• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
• Pediatric Advanced Life Support preferred. American Heart Association or American Red Cross accepted.

Education:

• Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.

What additional preferences we're seeking

Prefer Pediatric w/some Ambulatory experience. Respiratory experience is also strongly preferred.
