Job Details: Job Title: CWHH-Physical Therapist Location: Charlotte, NC Setting: Home Health Pay Range: 2558.60 - 2686.53 Schedule: Discuss with recruiter. Contract Length: 13 weeks. Candidate Type: Traveler Requirements: Current State License as a Physical Therapist; Proof of current CPR certification; Current driver's license. Must have reliable transportation and insurance. Responsibilities: Perform clinical assessments to identify patient needs and appropriateness of care. Document, review, and complete a Plan of Care (POC), progress notes, phone orders, and other clinical record documentation; revise the POC based on evaluation and ongoing assessment data. Plan and provide appropriate treatment for neuromuscular, neurological, and orthopedic disorders based on medical referral and therapy assessments. Assist/instruct patients and families in transfer techniques appropriate to patient abilities; instruct on use of equipment such as walkers, wheelchairs, canes, braces, prosthetic devices, and appliances in a safe manner. Educate patients and families regarding therapeutic procedures considering age, condition, culture, and learning readiness. Oversee other team members in patient care, ensuring care is consistent with patient needs and desires. Apply infection control and universal precautions during patient care activities to prevent contamination and disease transmission.
