In this role, you’ll conduct in-person assessments, develop individualized care plans, and collaborate with physicians, social workers, and caregivers to close care gaps and improve outcomes. You will educate members and families on conditions, medications, and preventive care, while monitoring progress and adjusting plans as needs change. You’ll leverage Humana’s resources, data, and technology to support holistic, person-centered care and connect members with community services that enhance health and well-being.
