Field-Based Community Health Advocate
Molina Healthcare of Illinois
Molina Healthcare in Monroe, OH is hiring a Community Health Worker to support community-based member advocacy activities and help vulnerable members manage health care needs. You will engage with members in their communities, coordinate with care teams, and assist with benefits navigation, transportation, scheduling, and social services referrals.
