Hospice RN Case Manager

Hospice RN Case Manager - per diem As a vital member of our Athena Home Health & Hospice care team, you will plan, organize, and direct hospice care services to meet the needs of individuals and families within their homes and communities. Responsibilities Supervise hospice aides and provide professional nursing care by utilizing all elements of the nursing process Complete comprehensive assessments of patient and family to determine hospice needs and provide regular re-evaluations Develop and implement care plans that establish goals, based on nursing diagnosis, and incorporate palliative nursing actions Counsel patients and families in meeting nursing and related needs, and provide health care instructions as appropriate Act as Case Manager when assigned by Clinical Supervisor, assuming responsibility to coordinate patient care for assigned caseload Requirements Graduate of an accredited school of nursing One (1) to two (2) years of recent acute care experience in an institutional setting, and one (1) to two (2) years of recent experience in home care Active and unencumbered RN license in Connecticut CPR certification Bachelor's degree, with one (1) year of hospice care experience preferred Benefits Competitive and Weekly Pay Holiday Pay Career Advancement Opportunities Exclusive Employer Discount Program Benefits Available for Eligible Team Members Employer Paid Life Insurance 401(k) with Employer Match Vacation and Personal Time Health, Dental, and Vision Insurance We are an equal opportunity employer that values diversity at all levels.
