The Senior Community Care (SCC) program focuses on coordinating and delivering comprehensive clinical services to some of our highest-risk health plan members. As an Advanced Practice Clinician (APC) , you'll have the opportunity to make a significant impact by providing individualized care, partnering with interdisciplinary teams, and supporting patients through complex health needs. Primary Responsibilities

• Complete comprehensive patient assessments, including:
  • Medical history review
  • Medication reconciliation
  • Physical examinations and vital signs
  • Evidence-based screenings and point-of-care testing, as appropriate
• Support members during short-term sub-acute rehabilitation stays
• Maintain accurate and thorough clinical documentation, including ICD-10 coding
• Collaborate closely with interdisciplinary care teams to coordinate patient care
• Lead advanced illness and goals-of-care discussions
• Educate and counsel patients and caregivers to support informed health care decisions
