RN Clinical Liaison

New Century Hospice is seeking an RN Clinical Liaison to coordinate hospice referrals, educate patients and families, and support seamless transitions of care. This role conducts clinical assessments, explains hospice services and eligibility, and collaborates with hospitals, physicians, and facility partners to identify appropriate hospice candidates. The RN Clinical Liaison develops individualized transition plans, ensures accurate documentation, and communicates patient needs to the hospice care team. Success in this role requires strong clinical judgment, communication skills, and compassion for patients and families facing end-of-life decisions.
