Clinical Pharmacist – Home Infusion
Seeking an experienced Clinical Pharmacist to support home infusion patients and collaborate with physicians, nurses, and other healthcare professionals.
Schedule: M–F, rotating 8–5, 8:30–5:30, or 9–6
On-Call: Friday 5:30 PM–Monday 8:30 AM, typically once/month
Travel: 0%
Responsibilities
- Evaluate and monitor drug therapy for home care patients.
- Review and verify patient orders, referrals, and medications.
- Provide medication consultation to patients and healthcare providers.
- Identify and resolve potential drug-related problems.
- Compound sterile medications, including antibiotics, TPN, chemotherapy, hydration, and other infused therapies.
- Maintain clean room and compounding areas in compliance with USP <797>/<800> .
- Maintain accurate clinical and pharmacy documentation.
- Oversee pharmacy technician functions and support quality assurance initiatives.
Qualifications
- BS in Pharmacy required; PharmD preferred
- Active Pharmacist license in state of practice
- 2+ years acute-care OR 1+ year home infusion experience
- Infusion drug compounding experience required
- Experience monitoring drug therapy for home care patients
- Strong communication, organization, and time-management skills
- Ability to participate in scheduled evening/weekend on-call coverage
