Registered Dietitian – Skilled Nursing Facility | Haskell, NJ

A premier 200+ bed Skilled Nursing Facility in Haskell is seeking a compassionate and dedicated Registered Dietitian to join our healthcare team.

The Registered Dietitian will be responsible for delivering high-quality nutritional care to residents, including:

• Completing new admission, re-admission, quarterly, annual, and significant change assessments

• Monitoring and documenting resident weights, weight changes, pressure ulcers, and wound care nutrition needs

• Managing nutritional care for hemodialysis and enteral feeding residents

• Completing MDS documentation

• Updating care plans and participating in interdisciplinary care plan meetings

• Reviewing and updating resident meal and snack preferences, entering diet orders, and assisting with menu planning as needed

• Conducting meal rounds and nutrition audits

• Providing nutrition education and counseling to residents and families

• Attending morning clinical meetings as required

• Maintaining strong communication with physicians, nurse practitioners, nursing staff, and the interdisciplinary care team

• Performing additional related duties as assigned

• Active Registered Dietitian license in the state of New Jersey required

• Long-term care or short-term rehabilitation experience preferred

• Experience with PointClickCare EMR preferred

• Experience with MealTracker preferred

• Ability to make independent clinical decisions

• Strong understanding of nutritional service regulations, standards, and best practices within long-term care settings

• Excellent verbal and written communication skills
