Wabash Senior Living & Rehabilitation is seeking a compassionate and motivated Physical Therapist (PT) to join our in-house rehabilitation team.
This is a great opportunity for a therapist who wants to build meaningful relationships with residents while helping them improve mobility, regain strength, increase independence, and reach their highest level of function.

What You’ll Do
- Evaluate residents and develop individualized plans of care
- Provide skilled physical therapy services based on each resident’s needs and goals
- Help residents improve strength, balance, mobility, and functional independence
- Monitor progress and adjust treatment plans as appropriate
- Collaborate with nursing, physicians, therapy staff, residents, and families
- Complete accurate and timely clinical documentation
- Maintain compliance with applicable state and federal requirements
Qualifications
- Current Illinois Physical Therapist (PT) license
- Strong communication and interpersonal skills
- Compassionate and resident-focused approach
- Ability to work effectively as part of an interdisciplinary team
- Skilled nursing or rehabilitation experience preferred, but not required
- New graduates are encouraged to apply!
