Lead Community Outreach & ECM Care Coordinator

Los Angeles Christian Health Center is seeking an Outreach Care Coordinator to support its mission by leading care management for the Medi-Cal ECM program, coordinating biopsychosocial needs and expanding patient enrollment. You will manage an ECM caseload, apply motivational interviewing, facilitate referrals, assist with scheduling, and engage with community partners to promote services.
