Seeking a licensed Physical Therapist to deliver rehabilitative services to patients in a skilled nursing facility. The role involves providing individualized therapy plans and enhancing patient recovery outcomes.

Responsibilities:

• Assess patients' physical conditions and develop tailored treatment plans.
• Implement therapeutic interventions utilizing advanced therapy equipment.
• Collaborate with interdisciplinary teams to coordinate patient care.
• Document patient progress and adjust therapy approaches as needed.
• Educate patients and families on therapeutic exercises and post-discharge care.

Qualifications:

• Valid state licensure as a Physical Therapist.
• Proven experience in a skilled nursing or similar facility setting.
• Strong knowledge of therapeutic techniques and equipment.
• Excellent communication and documentation skills.
• Ability to work full-time, Monday through Friday, with occasional weekends.
