Job Summary

QuickPro Staffing is seeking a Registered Physical Therapist (PT) to provide physical therapy services to patients in their homes throughout Brown County and Clermont County, Ohio.

The Physical Therapist provides skilled therapy services on an intermittent basis in accordance with physician orders and the patient's individualized plan of care. The PT will work under the direction and supervision of the Director of Clinical Services/Clinical Manager and collaborate with other members of the home health team to provide quality patient care.

Qualifications

• Graduate of an accredited Physical Therapy program.

• Current and unrestricted Physical Therapist license in the state of Ohio.

• Two years of experience preferred.

• Previous home health experience preferred.

• Ability to work independently and exercise sound professional judgment.

• Strong communication, organizational, and documentation skills.

• Commitment to providing compassionate, patient-centered care.

• Valid driver's license and reliable transportation required for travel between patient homes.

Responsibilities

• Provide physician-prescribed physical therapy services according to the patient's plan of care.

• Evaluate patients and assess their level of physical function.

• Develop, implement, and modify individualized plans of care.

• Perform initial assessments and reassessments as required.

• Develop and update maintenance therapy programs when appropriate.

• Help patients improve mobility, strength, function, and independence.

• Work to improve or minimize residual physical disabilities.

• Perform skilled therapy procedures as ordered by the physician.

• Monitor patient progress and communicate changes in condition or treatment needs.

• Consult with physicians and other healthcare professionals regarding changes in patient condition or treatment.

• Provide progress reports to physicians as required.

• Educate patients and family members on home exercise programs, safety, mobility, and activities of daily living.

• Participate in patient care planning and care conferences.

• Direct and supervise Physical Therapist Assistants and Home Health Aides as assigned.

• Collaborate with other home health personnel to coordinate patient care.

• Complete accurate clinical documentation and progress notes in a timely manner.

• Complete required OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant change-in-condition documentation according to agency requirements.

• Appropriately utilize ICD-10 codes.

• Participate in Quality Assessment and Performance Improvement (QAPI) activities as assigned.

• Participate in in-service training and educational programs as required.

• Maintain compliance with all agency policies, procedures, and applicable regulations.

Working Environment

• Patient homes throughout Brown County and Clermont County, Ohio

• Agency office as needed

• Regular travel between patient homes
