Family Nurse Practitioner Opening Outside of ProvidenceJohnston, RIFull TimePermanent PositionSeeking BC/BE Family Nurse Practitioner to join primary care team in Johnston, Rhode IslandPosition Highlights:Federally Qualified Health Center (FQHC) settingPatient-Centered Medical Home (Level III) modelOutpatient primary care practiceTeam-based, collaborative care environmentFocus on underserved and community-based populationsEmphasis on care coordination, prevention, and population healthResponsibilities:Provide comprehensive primary care servicesManage acute and chronic conditionsCoordinate care with interdisciplinary teamSupport patient self-management and educationParticipate in quality improvement initiativesMaintain accurate and timely documentationQualifications:Graduate of accredited Nurse Practitioner programBoard Eligible or Board Certified in Family PracticeActive Rhode Island NP license and DEA or ability to obtainExperience in PCMH setting preferredCompensation & Benefits:Competitive salaryMedical, dental, vision, life, and disability insurance403(b) retirement plan with employer matchPaid time off including vacation, sick, and holidaysContinuing medical education (CME) allowancePaid malpractice insuranceLoan repayment eligibilityProfessional development and training opportunities
