Job Description
Title
Case Manager RN - Acute Care

Schedule
Shift: 5x8 Days (Monday-Friday business hours); shift times may be one of the following: 0700-1530, 0730-1600, 0800-1630, or 0830-1700.

Assignment Details
Guaranteed Hours: 72 hours per 2 weeks (clinician may be called off for one 8-hour shift per 2 weeks).

Requirements
- AK RN license required for auto-offer (pending license may be acceptable for submission)
- Acute care hospital case management experience required
- Preferred: ACM-RN certification, BLS certification
- Experience with Epic EMR is required
- Familiarity with utilization review tools, such as InterQual and Milliman, preferred

Roles & Responsibilities
- Coordinate care and discharge planning for patients across the care continuum
- Conduct continued stay reviews and perform medical necessity determinations
- Collaborate with interdisciplinary teams including physical therapy, respiratory therapy, pharmacy, social services, and others
- Ensure compliance with regulatory standards and hospital policies

Compliance / Notes
- Must be willing to work every other weekend and 2 of 3 major holidays (Thanksgiving, Christmas, New Year)
- Orientation: Approximately 24 hours
- Parking is free
- Scrub color will be provided
- Only 1 time-off request per 13-week assignment (5 days or less); no holiday time-off requests for auto-offer
