Are you ready to make a meaningful difference in the lives of individuals facing health challenges? Our Home Health team is seeking a goal-oriented RN to work with patients in their home environments.

As a member of our dedicated team, you'll:

• Deliver high-caliber care directly to individuals within the comfort of their homes in a designated geographic territory, while also overseeing case management to ensure comprehensive fulfillment of clients' needs.
• Foster collaborative partnerships with colleagues, local physicians, and community providers.
• Establish goals of care and call on the full range of clinical and communication skills to help your clients reach their goals.

Vermont Licensed RN with a passion for providing home health care.

Prior home health experience and/or case management background preferred, though comprehensive training and orientation will be provided.

Valid VT driver's license and reliable vehicle with automobile insurance is essential for home visits.

Ability to manage time and work independently
