Case Manager

We’re looking for a Hybrid Case Manager to join our housing team and support shelter residents as they build stability and self-sufficiency. In this role, you’ll work with participants at our Jonathan’s Place shelter, which provides services for individuals transitioning out of homelessness. This role assists residents with integrating into the shelter, navigating challenges, creating goals for progress and moving toward greater independence while maintaining a clean and healthy living environment. Through consistent, onsite support, you’ll play a key role in ensuring participants feel supported, seen, and genuinely set up for both short- and long-term success. Education or Training Equivalent: Bachelor’s degree in social work, psychology, social welfare, or a related field or two (2) years of experience in a related residential or housing program. Qualifications and Abilities: Minimum of two (2) years’ experience working with under-resourced or low-income populations; experience working with individuals experiencing homelessness unhoused populations preferred. Experience in residential and/or housing programs. Knowledge of Housing First, harm-reduction and trauma-informed principles. Working knowledge of SSI, SSDI and other entitlement programs. Experience with data entry systems (HMIS, Salesforce, or similar). Proficiency with Microsoft Word, Excel, cloud-based systems, and Adobe. Comfort working in a culturally and ethnically diverse environment. Physical Requirements: Ability to sit and use a computer for moderate periods (1–3 hours per day). Ability to stoop, crouch, kneel, or crawl occasionally. Ability to lift or move objects up to 40 pounds. Manage a caseload of up to 8 individuals with intensive and outcome driven supportive services while delivering these services utilizing a Housing First approach. IncorporateCalAIMcare coordination and housing stabilization strategiesinto individualized housing and care plans, in collaboration with healthcare, behavioral health, and community-based partners. Provide housing-focused case management in one‑on‑one and group settings. Identifyand address barriers to housingstabilization, including chronic homelessness, mental and physical health needs, substance use disorders, under‑employment, lack of income, and other systemic barriers. Skillfully prevent and/or resolve conflictsskillfullyusinga strength‑basedand trauma‑informed approach. Respond to urgent and crisis situations, including coordination with EMTs, fire, law enforcement, psychiatric services, and community partners as needed. Provide supportiveservices, guidance, and referrals to community resources. Engage program participants regularly and consistently to support program participation and progress toward housing stability while remaining flexible and creative around interventions. Coordinate closely with the County behavioral health team working onsite and attend weekly care conference meetings. Assist with facilitating regular activities on‑site that support housing focused programming. Collect monthly rent checks and log payments in agency database Provide bus tickets, make change for laundry and support activities of daily living while responding to requests for support from tenants as needed. Support regular cleaning on housing floors while ensuring all shared spaces meet agency cleanliness standards Facilitate maintenance needs in collaboration with Program Manager and maintenance crew. Communicate tenant concerns to Program Manager in a prompt and timely manner. Perform other duties as assigned Documentation and Compliance: Completetimely,accurate, and compliant documentation, including case notes, assessments,careplans, and progress updates,in accordance withpolicies and funder requirements. Ensure client records clearlydemonstrateeligibility, service delivery, engagement, and progress toward housinggoals. Maintainaccurateandtimelydata entry inHIPPO (Salesforce database)andHMIS. Meet all documentation and case management deadlines, including intake, enrollment, reassessments, housinggoals, and discharge documentation. Participate in quality assurance, monitoring, and audit processes, including responding to documentation reviews and corrective actions. Case Management, Housing-Focused Services and Outcomes: Develop, implement, and regularly update individualized, client-centered housing and care plans. Monitor, document, and adjust plans proactively to address changing needs or barriers. Coordinate services with internal teams and external partners to ensure continuity of care andtimelyreferrals. Document housing and service outcomes to support program performance measurementsandgrantreporting. Professional Standards and Team Collaboration: Manage a hybrid caseloadandbi-weekly/monthlymeetings,with strong organization, time-management, and prioritization skills. Communicate clearly and professionally with supervisors, team members, volunteers, and program participants. Participate in weekly supervision, program meetings, all-staff meetings, and requiredtraining. Maintain confidentiality andcomply withall agency policies, including HIPAA where applicable. Contribute positively as a collaborative, accountable member of the Homeward Bound team. Desired Qualities: Compassionate, flexible, non-judgmental, and respectful.
