Clinical Liaison

What You’ll Do Act as the primary point of contact for patients transitioning from hospital to home infusion, ensuring a smooth and supportive experience Coordinate the initiation of Parenteral Nutrition (PN), including patient assessments, information gathering, and care planning Build and maintain strong referral relationships with physicians, hospitals, and key decision-makers Partner closely with sales, clinical, and operations teams to grow referrals and streamline patient onboarding Educate patients and caregivers on therapy, equipment, and self-care best practices Provide clinical insight and support to healthcare providers, including therapy recommendations and patient-specific considerations Support bedside teaching and assist team members across markets as needed Track activity and manage opportunities using CRM tools Collaborate with marketing on outreach initiatives and promotional efforts Maintain accurate documentation of patient interactions and care plans Drive territory growth and meet or exceed performance expectations Travel throughout the assigned territory (and occasionally beyond) to support patient care and business growth Requirements Active Registered Nurse (RN) license in current state (or compact license) Strong clinical understanding, ideally within infusion, nutrition support, or transitional care Experience building relationships with providers, hospitals, and referral sources Ability to educate patients, caregivers, and healthcare professionals effectively Comfort working cross-functionally with sales, operations, and clinical teams Strong organizational skills with the ability to manage multiple patients and priorities Experience using CRM tools to track activity and performance Valid driver’s license and proof of auto insurance Willingness to travel within territory and occasionally on short notice #J-18808-Ljbffr The Remedy Group
