• Evaluating patients in various settingsbe it inpatient or outpatientand crafting personalized treatment plans tailored to their unique needs.
• Administering physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function.
• Collaborating closely with nursing staff, physicians, case managers, and other rehabilitation professionals to optimize patient outcomes.
• Providing education to patients and their families regarding therapeutic objectives, discharge plans, and at-home exercises.
• Meticulously documenting evaluations, treatments, and progress in line with facility and regulatory standards.
• Actively engaging in care team meetings and interdisciplinary conferences to ensure cohesive and coordinated patient care.

Regarding qualifications, we seek passionate and driven individuals with:

• A Doctorate in Physical Therapy (DPT) or equivalent degree from an accredited institution.
• State licensure as a Physical Therapist is mandatory, and while CPR certification is preferred, having any specialized certifications, such as neurological or geriatric, will certainly be advantageous but not a requirement.
• Demonstrated previous experience, preferably in a clinical setting.
• A comprehensive understanding to treat patients dealing with complex medical and mobility challenges.
• Strong interpersonal and communication abilities, along with being at ease with digital charting and clinical documentation systems.
