About Groove Most people with osteoporosis, arthritis, or chronic pain get a diagnosis and a pamphlet. Maybe a referral to PT for six visits. Then they're on their own. Groove exists to close that gap. Groove is a remote, PT-led program that gives older adults with chronic musculoskeletal conditions an ongoing relationship with a clinician who knows them, meets them where they are, and helps them get stronger over time. We're early-stage, small, and building the kind of care most clinicians got into this field to provide. The Role We're hiring a California-licensed PT to own a caseload of patients and work with them over weeks and months, not visits. You'll build trust, design programs around their lives, and use new tools to deliver care in ways most PTs haven't experienced yet: AI, daily communication, remote monitoring, and telehealth. You'll work alongside our Founding Clinician, collaborate with our physician team, and report directly to the CEO. What You'll Own Patient Onboarding & Care Conduct initial intake calls to understand each patient's goals, history, and limitations Build individualized, evidence-based programs focused on strength, balance, and bone health Progress programs over time based on patient response and load tolerance Ongoing Monitoring & Engagement Communicate regularly with patients (primarily via SMS) to support, guide, and maintain momentum Conduct periodic video check-ins to assess progress and deepen rapport Adapt programs in real time secondary to patient feedback Drive adherence through consistent touch points and support Who You Are Passionate about this population. Older adults, bone health, and longevity matter to you. Clinically sharp. Evidence-based, analytically strong, and comfortable building effective programs. A natural with people. You build rapport quickly and genuinely enjoy the relationship side of care. You love programming. Designing strength and movement programs is energizing to you — not just clinically, but personally. You care about exercise, you think about load and progression, and you get excited about helping people get stronger. Curious and adaptable. You're excited to learn new tools and practice in ways most PTs haven't yet. Low-ego, high-ownership. Background 2-3 years of clinical experience, with: Populations: orthopedics, older adults, osteoporosis, or geriatric rehab Settings: outpatient, home health, telehealth, or digital health Roles: independent caseload management; startup or remote care experience a plus Credentials: PT or DPT; active California license required What This Role Is (and Isn't) This is not a traditional clinic role. It’s a flexible, remote role where you own patient outcomes and build meaningful longitudinal relationships. You'll have autonomy in how you work, with support from a small, high-caliber team. If you need a structured clinical environment to do your best work, this probably isn't the right fit for you.
