This position offers flexibility and meaningful patient interactions by conducting thorough, patient-centered health risk evaluations for Medicare Advantage members throughout Shelby County. Schedule:

• Flexible shifts available Monday through Sunday
• Design your own work schedule
• Set your weekly or bi-weekly hourly commitments

Key Responsibilities:

• Perform annual home health risk assessments for Medicare Advantage members
• Gather and review comprehensive patient medical histories and medication regimens
• Educate patients on medical conditions and appropriate medication usage
• Identify care gaps and provide actionable recommendations for optimal health outcomes
• No requirement for treatment plan creation or prescribing of medications

Qualifications:

• Active Nurse Practitioner License in the state of Ohio
• Up-to-date CPR certification (BLS)
• Board Certification through AANP or ANCC
• Minimum of 6 months current NP experience preferred
• MSN degree from an accredited nursing program

Compensation for this opportunity is highly competitive, with $80-$90 per evaluation, plus a mileage stipend and additional compensation for administrative time.
