Foundation Living LLC is seeking experienced Housing Support Professionals to join our growing team serving Medicaid members throughout New Jersey. We are currently hiring three full-time team members with experience in housing navigation, tenancy support, supportive housing, homelessness services, behavioral health, case management, or community-based social services.
Position Summary
The Housing Support Professional will assist individuals experiencing housing instability by providing pre-tenancy and tenancy-sustaining services. Staff will work directly with Medicaid members to secure safe, stable housing and maintain successful community living.
This is a Hybrid position.
Responsibilities
• Conduct housing assessments and identify barriers to obtaining or maintaining housing.
• Develop individualized housing support plans.
• Assist members with housing searches, landlord outreach, housing applications, and lease acquisition.
• Coordinate with property managers, landlords, behavioral health providers, healthcare providers, and community organizations.
• Provide tenancy-sustaining services including budgeting, life skills coaching, conflict resolution, and crisis intervention.
• Maintain accurate documentation and complete required service notes.
• Participate in interdisciplinary care team meetings.
• Connect members to community resources and supportive services.
Minimum Qualifications
• Bachelor’s degree preferred in Social Work, Human Services, Psychology, Sociology, or related field.
• Minimum two years of experience providing housing services, supportive housing, homelessness services, behavioral health case management, care management, or similar community-based services.
• Experience working with individuals experiencing homelessness, serious mental illness, substance use disorders, or other vulnerable populations preferred.
• Strong organizational and communication skills.
• Valid driver’s license and reliable transportation.
