Clinical Supervisor OHC– Franklin & Surrounding Counties Company: CareStar, Inc. We are currently seeking a Clinical Supervisor to join our Ohio Home Care program. This is a meaningful opportunity for professionals who are passionate about helping others live healthier, more independent lives. As a Clinical Supervisor , you’ll work directly with individuals to assess their needs, develop personalized care plans, and connect them with essential services and supports. You’ll be part of a mission-driven team that values your expertise, supports your growth, and empowers you to make a real difference in your community. Key Responsibilities Knowledge of Medicare, Medicaid, insurance and waiver programs and community resources. Good problem-solving skills. Integrity and self-discipline with the ability to work independently within guidelines. Understands and complies with CareStar Policies and Procedures. Maintains confidentiality as related to patient information. Follows the Acceptable Use Policy while using any information systems owned or controlled by CareStar, Inc. Adheres to the CareStar Rule in performance of job responsibilities. Minimum Qualifications Experience providing care management/coordination preferred. Must be either licensed RNs, licensed social workers or licensed independent social workers (LSW, LISW) with at least five (5) paid years of clinical experience in home-and community based services and a minimum of one (1) year of supervisory experience. Certified Case Manager (CCM) required or obtained within one year of being in the position. Experience in completing assessments, service plan development and monitoring. Effective organizational and communication skills. Experience providing care management/coordination preferred. Why Join CareStar?
