Clinical Pharmacist - Inpatient

SUMMARY Reports to the Pharmacist-in-Charge. Under general supervision of a Pharmacy Supervisor, is responsible for the interpretation, evaluation, preparation and dispensing of medications pursuant to a legitimate order in keeping with established policies, procedures, and standards of practice and scope of pharmacy services of the respective affiliate. Collaborates with multidisciplinary care providers to monitor, assess, and intervene as needed to ensure safe, appropriate, and cost‑effective medication use. Oversees and closely reviews the work of pharmacy technicians and ancillary support personnel. RESPONSIBILITIES Know and act in accordance with the Brown University Health Corporate Compliance Program and Code of Conduct. Receive patient medication orders as written by a physician or practitioner and review each order for completeness, therapeutic appropriateness, and consideration of patient factors. Contact prescribers to initiate interventions that ensure therapeutic efficacy, minimize risks, and optimize cost‑effectiveness. Apply pharmacotherapeutic and pharmacokinetic principles to ensure safe and effective medication use for the patient populations served. Provide drug information and consultation to prescribers, nurses, and other health care professionals regarding drug interactions, side effects, incompatibilities, allergies, discrepancies, stability problems, and drug administration. Proactively review and approve medication orders in LifeChart, ensuring relevant patient parameters are present. Supervise pharmacy interns and technicians, ensuring performance standards and adherence to federal, state, and organizational regulations. Identify, document, resolve, monitor, and prevent drug‑therapy related problems such as contraindications, duplications, and interactions. Engage with other clinical pharmacists for complex clinical situations and integrate patient‑focused medication therapy management per established pathways. Participate in prospective and retrospective pharmacokinetic dosing and monitoring, recommending dose adjustments and laboratory monitoring. Ensure compliance with target drug programs, formulary management initiatives, medication management initiatives, and adverse drug event monitoring programs. Document and report all clinical interventions, medication errors, near misses, and adverse drug events per protocols. Participate in emergency code blue response. Attend and participate in daily medical, multi‑disciplinary, and specialty rounds as time allows. Review and approve formulations for sterile products requiring compounding or manipulation, performing in‑process and final checks for accuracy, sterility, and stability. Participate in education and training of health care practitioners, including residents, students, interns, technicians, physicians, practitioners, nurses, and other professionals on medication use. Serve as preceptor to pharmacy residents and students. Participate in continuous quality improvement activities and total quality management initiatives, including adverse drug reaction, pharmacist clinical intervention, and medication incident reporting programs. Coordinate transitions of care, as appropriate, and maintain professional competence through continuing education and professional organization participation. QUALIFICATIONS Basic Knowledge Bachelor of Science in Pharmacy or Doctor of Pharmacy degree from an accredited college of pharmacy. Current unrestricted licensure as a licensed pharmacist or a licensed 90‑day temporary pharmacist in Rhode Island at start date; obtain full licensure before the end of temporary period. Current American Heart Association Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) certifications within 120 days of hire. Experience Successful completion of Post Graduate Year 1 (PGY‑1) Residency or national Board Certification in Pharmacotherapy preferred; PGY‑2 Residency in a specialty strongly preferred where applicable. Clinical experience, knowledge, and development relative to the role; may be considered in lieu of formal training. Effective interpersonal, written, and verbal communication skills. Effective organizational and presentation skills. Knowledge of area/specialty specific technology, including pharmacy information systems, automated dispensing technology, software, equipment, and automation. Experience with MS Word, Excel, Outlook, and web browsers. WORK ENVIRONMENT AND PHYSICAL REQUIREMENTS Extended periods of standing and walking; requires visual and manual dexterity to operate a computer. Occasional travel between facilities or sites as required. INDEPENDENT ACTION Functions independently within policies and practices; refers specific complex problems to supervisor when clarification is required. SUPERVISORY RESPONSIBILITY None.
