The goals of the Certified Community Behavioral Health Clinics program in MN are to 1) increase access to community-based mental health and substance use disorder services (particularly to under-served communities), 2) advance integration of behavioral health with physical health care, and 3) improve utilization of evidence-based practices on a more consistent basis.

Position Summary:
The School-Linked Behavioral Health Practitioner and Targeted Case Manager plays a vital role in supporting students' emotional and behavioral well-being. This position is responsible for providing skills training, targeted case management, crisis intervention, and treatment plan development and review. The practitioner also administers and reports standardized assessments to monitor progress and guide care. Working closely with school staff, families, and community partners, the practitioner ensures coordinated, student-centered support aligned with individualized treatment goals. This role may be based in contracted schools, RMHC office sites, or the community/home of the client and may require travel between sites.
Duties/Responsibilities:
- Provide individual, group, and family skills training at assigned school site(s), clients’ homes, or within the community.
- Instruct, assist, empower people served to develop and enhance stability, competencies, personal/emotional adjustments and independent living and community skills.
- Provide direct client case management support as assigned.
- Complete prior authorizations as needed for services; ensure insurance is active, and assigned clients are eligible for services monthly.
- Assist with scheduling and compliance with regards to appointments.
- Coordinate School-Linked Behavioral Health Services at assigned school site(s).
- Build and maintain positive working relationships with partnering agencies.
- Provide school/community personnel with essential program information.
- Respond to and provide crisis assistance and intervention as needed.
- Provide consultation and educational services to school/community authorized people.
- Complete all clinical documentation and other medically necessary information in the RMHC electronic health record for the people we serve both as required by agency policy and DHS standards in a timely manner.
- Develop and update individual treatment plans as required.
- Complete functional assessments (FA), Individual and Family Community Support Plans (IFCSP), and crisis prevention plans (CPP) as required.
- Complete required standardized measurement tools for children and adolescents.
- Provide medically necessary behavioral health treatment to children, adolescents and their families, as directed by RMHC Code of Conduct and Minnesota regulations.
- Attend Multidisciplinary, Special Education, school, and other meetings as directed.
- Coordinate care and referrals with other service agencies and families as needed for the people we serve.
- Provide program transportation as necessary.
- Attend training(s) as required by RMHC and the Department of Human Services to meet minimum training requirements and mandated training.
- Provide notification to direct supervisor of work schedule; be able and willing to work on the assigned full-time equivalency ratio and to meet the needs of the school district(s) assigned.
- Ability to identify, understand, and respond to the needs of clients and advocate for the people we serve.
- Communicate with Clinical Supervisor regarding issues/updates for the people we serve as required by the Department of Human Services and RMHC.
- Follow all pertinent regulatory and statutory standards and guidelines with the Department of Labor and all RMHC policies and procedures.
- All other duties as assigned.
Required Skills/Abilities:
- Willingness and ability to complete training in cultural competencies and provide culturally responsive treatment to youth and their families.
- Ability to deliver services at or above the agency’s expected level of productivity.
- Strong interpersonal skills to build and maintain collaborative relationships with partnering agencies.
- Possession of a valid driver’s license and insurance, a clean driving record, and access to reliable transportation.
- Flexibility to adapt to the demands of a dynamic and changing work environment.
- Strong organizational, time management, and crisis management skills.
- Knowledge of local community resources and state mental health resources and systems.
- Proficiency in providing individual, group, and family services in alignment with CTSS (Children’s Therapeutic Services and Supports) standards.
- Understanding and knowledge of records and billing compliance.
- Computer skills are sufficient to utilize email and accurately document services into an electronic medical record.
- A non-judgmental attitude and a client-centered approach to care.
- Ability to work independently with minimal supervision, as well as collaboratively within a team environment.
- Effective verbal and written communication skills in English.
- Ability to communicate professionally and respectfully with coworkers, clients, and external stakeholders.

Education and Experience:
- An individual who is qualified in at least one of the ways described in paragraph (b) to (d) may serve as a behavioral health practitioner.
(b) An individual is qualified as a behavioral health practitioner through relevant coursework if the individual completes at least 30 semester hours or 45 quarter hours in behavioral sciences or related fields and:
(1) has at least 2,000 hours of experience providing services to individuals with:
(i) a mental illness or a substance use disorder; or
(ii) a traumatic brain injury or a developmental disability, and completes the additional training described in section 245I.05, subdivision 3, paragraph (c), before providing direct contact services to a client;
(2) is fluent in the non-English language of the ethnic group to which at least 50 percent of the individual's clients belong, and completes the additional training described in section 245I.05, subdivision 3, paragraph (c), before providing direct contact services to a client;
(3) is working in a day treatment program under section 256B.0671, subdivision 3, or 256B.0943;
(4) has completed a practicum or internship that (i) required direct interaction with adult clients or child clients, and (ii) was focused on behavioral sciences or related fields; or
(5) is in the process of completing a practicum or internship as part of a formal undergraduate or graduate training program in social work, psychology, or counseling.
(c) An individual is qualified as a behavioral health practitioner through work experience if the individual:
(1) has at least 4,000 hours of experience in the delivery of services to individuals with:
(i) a mental illness or a substance use disorder; or
(ii) a traumatic brain injury or a developmental disability, and completes the additional training described in section 245I.05, subdivision 3, paragraph (c), before providing direct contact services to clients; or
(2) receives treatment supervision at least once per week until meeting the requirement in clause (1) of 4,000 hours of experience and has at least 2,000 hours of experience providing services to individuals with:
(i) a mental illness or a substance use disorder; or
(ii) a traumatic brain injury or a developmental disability, and completes the additional training described in section 245I.05, subdivision 3, paragraph (c), before providing direct contact services to clients.
(d) An individual is qualified as a behavioral health practitioner if the individual has a master's or other graduate degree in behavioral sciences or related fields.
- Experience Preferred: One or more years of providing Targeted Case Management services and working with children.

- Ability to speak and articulate the English language clearly and understand
- Ability to maintain a professional level of personal hygiene and physical appearance
- Must be able to maintain a Minnesota driver's license and auto insurance
